Mayo Clinic contracts with hundreds of insurance plans, but not all plans are part of our large network. Out-of-network insurance plans are not required to pay Mayo Clinic for your care, except in certain circumstances, such as an emergency. This means you will be financially responsible for any balance not covered by your insurance.
Your insurance company may need a referral from your primary physician to optimize your out-of-network benefits. Please contact your primary care physician to initiate a referral and seek approval from your insurance company to be seen at Mayo Clinic.
Your physician's referral alone does not constitute approval from your insurance to be seen at Mayo Clinic. An authorization or referral from your insurance often is required in addition to your physician referral. Be aware that even with approval, you may still have higher out-of-pocket costs at Mayo Clinic than if you were cared for by an in-network provider due to accessing your out-of-network benefits. Insurance authorization is not a guarantee of payment. You will be responsible for any balance not covered by your insurance.
Mayo Clinic is committed to transparency about the cost of your care. For additional information, contact your insurance company.
Your next steps to obtain a referral from your insurance provider:
- Contact your primary care physician to ask that they submit an insurance referral to your insurance provider, asking for you to receive care at Mayo Clinic.
- They may ask for Mayo Clinic's Tax ID numbers; those can be found below.
- Obtain approval from your insurance provider to receive care at Mayo Clinic.
- Understand the approval dates and services covered under your authorizations.
- Call your insurance provider with questions about your benefits and authorizations.
If you have questions related to your health insurance plan's specific out-of-network benefits and in-network facilities, please contact your insurance provider directly.
Without an approval on file from your insurance company, you may be asked for a deposit before proceeding with unauthorized services.
