There are many commercial or traditional health insurance plans with which Mayo does not participate. If your insurance plan does not participate with Mayo Clinic, it is difficult to predict what will be paid. You must check with your employer, health plan or other appropriate party about benefits before being seen. It's important that you know your health plan benefits.
Disability insurance
Mayo Clinic will file your disability, waiver of premium or loan payment claims for you. Your doctor will determine the length of your disability based on your medical condition. Discussing specific details with your doctor may help speed the processing time of your claim.
Mayo Clinic's Arizona and Florida campuses: Please give the claim form to your provider.
Mayo Clinic's Minnesota campus: Please give the claim form to a Registration and Business Services representative at Admissions and Business Services, Gonda Building, Lobby Level. You may also send the form to the Recovery and Claims Services Disability Unit.
Motor vehicle insurance information (if services are accident related)
If your care is the result of a motor vehicle accident, please provide this information at the time of registration. If your motor vehicle insurance benefits are exhausted and you don't have general medical insurance to cover your visit at Mayo Clinic, please contact us to pay your bill or ask questions about your statement.

Prescription plan cards
After your care, your doctor may prescribe take-home medications. You may choose to fill your prescriptions at a Mayo Clinic pharmacy or a pharmacy of your choice.
Mayo Clinic pharmacies accept cash, checks or credit cards for your copayment. Please bring all current insurance cards and prescription drug benefit cards when visiting the Mayo Clinic Pharmacy.

Workers' compensation
Mayo Clinic does not typically accept new patients for the initial workers' compensation visit. Exceptions may be made for certain clinical areas only if a referral or insurance prior authorization is obtained.
Patients with workers' compensation in Ohio, Washington and West Virginia will be considered self-pay.
At the time of illness or injury, your employer is responsible for filing a report of injury with the Industrial Commission. Once the report is accepted, a claim number is issued.
Prior to the time of service, Mayo will contact the employer's insurance company to verify coverage.
When emergency room services are required to treat a work-related injury, Registration will require the workers' compensation insurance information, and completion of an Industrial 102 form.
Referral follow-up from Mayo Clinic Hospital Emergency Department will depend on your employer and insurance company guidelines. The insurance company chosen by the employer handles workers compensation benefits.
In the event the insurance company denies payment, the patient has the right to appeal to their state board and request a hearing. Upon receipt of the denial from the insurance company, Mayo will contact the patient for payment. Inquiries regarding claim status should be done through the insurance company or state compensation fund.
