Out-of-network noncontracted plans

Out-of-network insurance plans do not have to pay Mayo Clinic for your care, except in certain circumstances, such as an emergency. This means you will be financially responsible for any balance not paid by your insurance.

An out-of-network health plan requires you to be approved to be seen for services provided at Mayo Clinic. A physician's referral alone does not constitute approval from your insurance to be seen at Mayo Clinic. An authorization from your insurance also is required. This is sometimes called an insurance referral. This insurance referral must be initiated by your primary physician to your insurance provider. Your insurance provider will then need to authorize your services at Mayo Clinic to ensure coverage at Mayo Clinic. Insurance authorization is not a guarantee of payment, and multiple authorizations may be required should you continue to pursue care at Mayo Clinic.

Due to accessing out-of-network benefits, even with approval, you may have higher out-of-pocket costs at Mayo Clinic than you would if you received care with an in-network healthcare professional. You will be financially responsible for any balance not covered by your insurance. Your health plan may not consider any of the amount you pay for care to count toward your deductible or out-of-pocket limit. Financial assistance is not available to out-of-network patients for nonemergency care.

You can contact your health plan to identify an in-network healthcare professional and facility to reduce your out-of-pocket liability.

How to obtain a referral from your insurance provider

  1. Contact your primary healthcare professional to ask that an insurance referral be submitted to your insurance provider to request that you receive care at Mayo Clinic. The insurance provider may ask for Mayo Clinic's tax ID numbers. Those can be found in the next section.
  2. Contact your insurance provider and obtain approval to receive care at Mayo Clinic.
  3. Confirm the approval dates and services covered under your authorizations.
  4. Call your insurance provider with questions about your benefits and authorizations.

Without an approval on file from your insurance company, you may be asked for a deposit before proceeding with unauthorized services.
