Requesting a change to your access

Contact the Mayo Clinic Referring Physician Office at carelinkservices@mayo.edu if you need any of the following changes to your access:

• You currently have access to Mayo Clinic CareLink for an organization and wish to add access for an additional organization.
• You no longer need access to Mayo Clinic CareLink.
• You would like to update any of the following information that was provided when you enrolled for access: Name, email address, phone number, Specialties, Credentials, NPI, home address, email address / phone / fax at your organization.
• You would like to have your password reset so that you can select a new password.
