Step 1:

Check to see that you are enrolled in an eligible group to receive your enhanced benefits at Mayo Clinic's campuses in Phoenix and Scottsdale, Arizona.

Contact Blue Cross and Blue Shield of Illinois at 888-802-8776 (toll-free) to get more information on the Boeing Centers of Excellence benefit and determine eligibility requirements.

Step 2:

Contact your Mayo Clinic navigator directly at 877-769-9191 (toll-free). The navigator will preregister you during this call, and you will use this number should you have any questions before, during or after your stay.

Step 3:

You, your home physician and your Mayo Clinic navigator will work together to coordinate all pre-surgical requirements before you come to Arizona. Mayo Clinic will need a copy of your medical records including copies of your radiology images. You and your home physician will need to complete the documents below:

Step 4:

After you have worked with your Mayo Clinic navigator to schedule your care at Mayo Clinic, you will need to notify BCBSIL online at BCBSIL or by phone at 888-802-8776 (toll-free) of your appointments and planned surgery or procedure date so that you can activate your Boeing Centers of Excellence program benefits.

Step 5:

With the assistance of your Mayo Clinic navigator, you will travel to Phoenix and meet your Mayo Clinic care team. We recommend the on-campus lodging at Residence Inn Phoenix Desert View at Mayo Clinic (www.marriott.com/phxmh). A complimentary shuttle is available to transport you to and from Mayo Clinic Hospital.

Once you arrive at Mayo Clinic Hospital, you will be greeted by a patient navigator in the hospital lobby. Your Mayo Clinic navigator will escort you to your first appointment and introduce you to your nurse care coordinator, and will be available to provide member escort services throughout your visit.

Step 6:

Your nurse care coordinator, along with the rest of your Mayo Clinic care team, will oversee your care until you are ready for discharge from the hospital. Your Mayo Clinic care team will plan your follow-up care and remain in touch with you throughout your recovery.

Step 7:

Your Mayo Clinic care team will communicate the discharge plan to your primary care physician and other providers before you visit them upon returning home and completing your follow-up care. Your referring physician may also access your records through a secure web-based portal.
