    To deactivate your account, call Mayo Clinic Customer Assistance at ‎1-877-858-0398.

    If you choose to 'Deactivate Account' your patient portal account will be deleted. This means you will lose access on both the web and mobile app. You will also lose access to any other patients you have linked to your account.

    You cannot reactivate your patient portal account once it has been deactivated. To regain access to your patient portal account, you will need to create a new account. To create a new account, please call:
