Mayo Clinic works with hundreds of insurance companies and is an in-network provider for millions of people. In most cases, Mayo Clinic doesn't require a physician referral. Some insurers require referrals, or may have additional requirements for certain medical care. All appointments are prioritized on the basis of medical need.

• Ask your local physician to refer you and arrange your appointment. The Referring Physician Number is 800-533-1564 (toll-free). This number is for use by physicians and their staffs only.

For Your Appointment

Please bring or send in advance the following items if they are available:

• All previous operative reports and pathology reports.
• Medical records and test results related to the reason for your appointment.
• A letter from your personal physician that summarizes your past treatment
