How does the Advanced Care at Home program work?
During your care, you're provided with the home care technology you need to be monitored remotely and to have virtual visits with the health care professionals on your team. You also receive in-home visits for services such as lab tests, mobile ultrasounds and X-rays, and IV therapies.
Why choose Advanced Care at Home?
Advanced Care at Home provides the hospital-quality care you need in the comfort and safety of your own home. The program includes 24/7 access to an expert care team at your fingertips, treatment and monitoring in your home, a care plan, a schedule designed around your needs, and supplies delivered right to your home.
Will my insurance pay for Advanced Care at Home?
Mayo Clinic works with your insurer on coverage for care provided. Insurance copayments, coinsurance and deductibles may apply.
What typical inpatient services can be provided in my home?
You can receive lab tests, IV therapies and mobile imaging such as ultrasounds and X-rays right in your home.
Who is on my care team?
Your care team is led by Mayo Clinic physicians and includes nurse practitioners, physician assistants, registered nurses, and a team of healthcare and service providers. Your care also may include in-home visits by a nurse practitioner, a physician assistant or other healthcare professionals, all as determined by your care team.
May I leave my home while in the Advanced Care at Home program?
During care, just as if you were in the hospital, we ask that you stay home. The care team considers your individual diagnosis and circumstances should you need to leave your home during your care.
Can I change my mind about being in the program?
At any time, you may ask to be discharged from the program. We will help transition your care to a safe alternative.
How does discharge work?
When you're well enough to be discharged, your Mayo Clinic care team ensures a smooth transition out of the program and the removal of all the home care technology.
What safety measures do you take?
The program has a thorough clinical approach to ensure your safety. Protections in place include:
- The ability to reach your clinical team anytime, day or night.
- A bracelet you wear that will dispatch 911 emergency services when activated.
- Monitoring equipment that reports information to your care team, such as blood pressure and heart rate.
