Membership fees

What is the fee for the program?

Membership in Mayo Clinic 365 requires an annual fee. This is in addition to costs for specific medical services. Pricing is based on the level of care you select. Please call 904-956-0006 to speak with a representative who can help you find the care plan that is right for you.

When is the fee due?

The initial fee is due with your membership agreement. The annual fee is billed on the anniversary of your membership.

Does Mayo Clinic bill my fee on a monthly or quarterly basis?

No. The fee must be paid in a lump sum on an annual basis to maintain your membership.

Does my insurance or health plan pay for the membership fee?

No. If you have a healthcare reimbursement account, it's possible the fee could be reimbursed by that account. Please check with the adviser who manages it.

Is the annual membership fee tax deductible?

Please check with your tax adviser for tax advice.

Do I have to pay the membership fee even if I don't use my doctor's services during that year?

Yes. The annual membership fee covers the cost of access to your doctor even if you don't use it.

Access and enrollment

How does Mayo Clinic provide more immediate access?

Mayo Clinic 365 doctors limit the number of people accepted into their practice. This ensures more time for your personalized, continuous care. As a member, you'll also have unrestricted 24/7/365 access to your concierge medicine doctor via cellphone or the user-friendly Mayo Clinic Patient Portal.

How can I enroll in Mayo Clinic 365?

Members are accepted on a first-come, first-served basis. Once each doctor reaches the limit on the number of people in their practice, the clinic starts a waiting list. To be accepted in the program, you must complete a membership agreement and pay your annual fee.

Does Mayo Clinic accept Medicare reimbursement?

Yes. We bill Medicare for covered services. Medicare doesn't reimburse for the membership fee.

Does Mayo Clinic participate in other insurance programs and health plans?

Yes. A list of our participating insurance plans and health plans is available upon request. The membership fee is in addition to charges for specific medical services.

Doctors

What are the credentials of these doctors?

All Mayo Clinic 365 doctors are board certified in internal medicine and are employed by Mayo Clinic. They were selected for their excellent reputations, commitment to service and demonstrated ability to build in-depth relationships with patients.

Is my doctor available to me while I'm traveling or out of town on an extended basis, including at my homes in other locations?

Yes. Your doctor is available to you by phone or portal message no matter where you are. When your doctor isn't available due to personal travel, one of their colleagues is available.

Medical services

What about the fees for tests, specialists and hospitalization?

Those charges are billed to your insurance, either private or Medicare, and are reimbursed according to your benefits. You are responsible for any fees not covered by your insurance.
