Referring a patient to Mayo Clinic Children's

Mayo Clinic Children's includes all Mayo Clinic locations — Minnesota, Arizona and Florida — and the Mayo Clinic Health System, with most services provided in Minnesota.

Call 855-MAYO-KID (855-629-6543, toll-free) and select option 2 to request a patient appointment, consult with a Mayo Clinic Children's staff member, or to directly admit or transfer a patient.
