  • Chronic disease management including diabetes, hypertension, heart failure, coronary artery disease, chronic pulmonary disease, depression, anxiety, and chronic pain
  • Preventative health care including cancer prevention, cardiovascular risk factor management, and obesity management
  • Undifferentiated symptoms and diagnostically challenging illness
  • Patient-centered care with an emphasis on partnership with a patient to develop individualized care plans that prioritizes the patient's goals in the context of their values, beliefs, preferences and living situation.
  • Longitudinal therapeutic relationships between the patient, the doctor and care team throughout a patient's life
