Get Started for Transplant

If you have been told that a transplant might help you, you may be eligible for enhanced benefits.
Step 1:
Check to see that you are enrolled in an eligible Walmart Inc. Associates' Health and Welfare Plan as your primary medical coverage:

Step 2:
Call your health care adviser at the number on the back of your medical plan ID card to begin the Centers of Excellence process. The adviser will first verify your plan eligibility, and then will obtain some basic information from you. Next the advisor will send your information to HealthSCOPE Benefits to get started.
Step 3:
HealthSCOPE Benefits will send you a packet of forms to complete. The forms include authorizations for your medical records to be sent to Mayo Clinic by each provider who has participated in your care.
Step 4:
Continue your current care path with your home physician while Mayo Clinic gathers your records. When Mayo Clinic schedules an onsite visit, you and a care companion may be eligible to travel to Mayo Clinic for evaluation and have your travel and lodging expenses covered.
