Get Started for Cancers

If you have received a diagnosis of blood, breast, colon, lung, prostate or rectal cancer, you may be eligible for enhanced benefits.

Step 1:

Check to see that you are enrolled in an eligible Walmart Inc. Associates' Health and Welfare Plan as your primary medical coverage:

Step 2:

Call your health care adviser at the number on the back of your medical plan ID card to begin the Centers of Excellence process. First, they will verify your plan eligibility, and second, they will obtain some basic information from you. They will send your information to HealthSCOPE Benefits to get started.

Step 3:

HealthSCOPE Benefits will send you a packet of forms to complete. The forms include authorizations for your medical records to be sent to HealthSCOPE Benefits by each provider who has participated in your cancer care. HealthSCOPE Benefits will gather your records and send them to Mayo Clinic for review.

Step 4:

Continue your current care path with your home physician while Mayo Clinic reviews your records. If Mayo Clinic recommends an on-site visit, you and a care companion may be eligible to travel to Mayo Clinic for evaluation and have your travel and lodging expenses covered.
